2. If you have additional members to add, select ADD FAMILY MEMBER on the next page.
3. If you require a log in today, please contact our office at 757-5626 (option 2) during business hours and we will assist you over the phone.

PLEASE NOTE: Requests for new household set up are received Monday through Friday with the exception holidays.
After completing this form, you will be contacted within 1-2 business days with your WebTrac username.

If you are updating your household information, enter current information and click FINISH.
